REQUIRED CAMP INFORMATION

Under the direction of the State of Oregon Health Authority, Coaches must fill out and submit a new form for each individual camp session. The information gathered is required for contact tracing per state guidelines.  As per the directive, this information will be kept for a 4 week period after your camp session and only used in the event that participants need to be notified.

    Please list each player along with the name of the parent dropping the player off/picking the player up. Please have current contact information for this parent for contract tracing purposes.
